Once CCHR has received your license agreement, your username and password will be e-mailed to you, so please be sure to clearly print your e-mail address and all information on your Licensing Agreement Form.
After you have received your e-mail from CCHR giving you your user name and password, return to this page to access the Basic Action Kit online.
